CAPriCORN Expands to Community Clinics for Population-Level Insights
The federated health record network, previously limited to hospitals, will now include community health centers—unlocking new data to track cardiovascular outcomes at scale.
CAPriCORN (Chicago Area Patient-Centered Outcomes Research Network) is expanding beyond its hospital roots to include community health centers for the first time—a move that will give Live Healthy Chicago unprecedented insight into blood pressure trends at the population level.
The federated electronic health record network allows researchers to analyze de-identified patient data across multiple institutions without centralizing sensitive records. Until now, it has primarily covered academic medical centers and large hospital systems.
“Adding community clinics changes everything,” says the project’s data lead. “These are the front lines of care. Now we can see what’s actually happening in the neighborhoods that need the most support.”
The expansion will enable the coalition to measure blood pressure control rates, medication adherence, and screening coverage in near real-time across all 27 priority neighborhoods.
Early analyses are expected by mid-2026, with quarterly dashboards planned for community stakeholders and partner organizations.
